ATHLETIC PHYSICAL INFORMATION- ATTENTION PARENTS
The time has come to get ready for the 2017-2018 school year. The Midlothian ISD Athletic Department requires all athletes participating in athletics to have a new athletic physical each year. This means that your son/daughter will need to get a physical before the new school year begins. To make it easier on you, the district will offer physicals on Saturday, May 20th. Athletic physicals through the district will only be done on this day, so please take advantage of it.
Physicals will be from 9:00 a.m. – 12:00 p.m. on Saturday, May 20th at Midlothian Heritage High School. Midlothian ISD has partnered with Mansfield Methodist Physician Network to conduct our athletic physicals this year. As a result, there will be approximately 12-14 male and female medical staff present for the physicals. We will need a parent/legal guardian to accompany the student in order to fill out the required paperwork. The cost of each physical is $20 and checks can be made payable to M.I.S.D.
In addition, Methodist Mansfield will offer free echocardiogram screenings. These free screenings are available by appointment only. Please call 682-242-6401 for more information on the free screenings.
All participants will beed to fill out and bring the following forms:
- Echocardiogram Screening Patient Consent Form
- Echocardiogram Patient Screening Evaluation
- Physical Evaluation Medical History Form
Those who do not have their physicals on May 20th may go to their own doctor anytime after May 1, keeping in mind that you must use the required U.I.L. physical form. The form may be picked up in the athletic office at Midlothian High School or obtained from the athletic website. Please understand that if you do not come on this day your son/daughter will need to get a physical before they can participate in athletics/pre-athletics for the 2017-18 school year.
If your child is under current restrictions due to a medical condition or currently under a doctor's care, then we would advise going to your primary care doctor for the physical.
Midlothian ISD Athletics will again be using electronic forms. All forms except for the physical/history form are done online. The physical/history form must still be printed and signed by the doctor/parent/athlete.
