Midlothian ISD Immunization Clinic

Prior to attending the clinic on August 4, you must complete the registration form linked here. You may also view the 2020-2021 immunization requirements for the State of Texas herePlease bring your child's shot record to the clinic to verify their vaccine needs.

Safety is of the utmost importance while providing these vaccines to our children therefore in accordance with Governor Abbott's executive order GA-29, masks will be required for any one over the age of 10. Hand sanitizer will also be provided for use before entering and proper social distancing will be enforced to prevent the spread of COVID-19.  

Incoming 7th Grade Students 
All incoming 7th graders are required to receive the Tdap and Meningococcal vaccine (on or after the child's 10th birthday) prior to the first day of school. This is for both virtual and face-to-face learners. 

If your student was missing any vaccines, your child's school nurse should have notified you prior to the end of the year, but don't hesitate to come to the clinic for verification.
