MISD employees do not have any additional step to take be seen at the clinic. However, dependent/spouse date must be updated in the MISD Third Party's Enroll System.
Individuals may not be seen at the clinic unless they are registered as a dependent/spouse of the employee in the system. During Open Enrollment that occurs each July, employees are asked to list all dependents and the spouse even when declining the coverage. Some employees have not listed all dependents and/or a spouse in the system. These individuals will not be eligible for the clinic UNLESS they are added to the system.
If you plan to use the clinic for your dependents and/or spouse and have not entered them in the system during Summer Enrollment, please use the attached form for the district to manually add them to the program (DOB/SSN are required). If dependents/spouse are updated in the system, no further action is required and the above form should not be completed.
